Decide the proposed new name of LLP and also intimate the reason for such change.
Apply in Form 1 to Registrar of Companies for change of name.
If the procedure for change of name is according to the agreement then provide for Extract of relevant provision of the Limited Liability Partnership Agreement, if any.
If LLP Agreement is silent about the procedure then take consent of all the partners for changing the name of LLP/ Minutes of decision/ resolution/ consent of the partners. Rule 20(1).
Give notice in Form 5 to Registrar for change in name within 30 days of such change. Rule 20(2).

Proposed name of LLP.
Reasons for change of name.
Copy of the minutes of the decision/ resolution/ consent of the partners.
Extract of relevant provision of the Limited Liability Partnership Agreement, if any.
